Kindergarten Registration is an online process at Fox Hollow   This allows you to fill out the forms online you are able to register at your convenience in the comfort of your home.  It reduces the amount of times you need to enter the same information more than once. It also addresses the concern for your, and our staff’s, health by eliminating lines and large groups of people gathering in one space.

Please note, completing the registration process online does does not finalize registration.  Read through and follow all instructions found at beginning of registration form.  We will print your completed registration forms so  you may then come into the office at your appointment time, with the required documentation (#3 listed below) and sign your paperwork.  All registration forms must be turned in and signed by a legal guardian, ie: custodial mother, father or court appointed guardian.

ALL  steps listed below must be complete before a time request can be considered:

1.  2021-2022 Kindergarten Registration Form

2.  TRANSPORTATION INFORMATION
      -Necessary for all students, even if they don’t ride the bus.

3.  Documents to Gather and Bring to School with you:

-Proof of Residence (current utility bill, mortgage statement, lease agreement)
*If living with others, completed and notarized form Declaration of Residency
*If living Out of Area, form must be completed and approved Out of Area
-Original Birth Certificate
-Immunizations to date (documentation from doctor’s office or health department) 21-22 Immunization Requirements
-Picture ID
-Guardianship/Custody Documents (if child does not live with both parents)

4. Time requests will open May 10th and will be considered only when all other steps are complete.
